CLEVELAND | July 22, 2025 — Tuesday Health, a value-based palliative care provider group, is marking the one-year anniversary of its partnership with CareSource by releasing a compelling new patient testimonial video that brings its mission to life. The video features Pam, a Cleveland-area woman living with multiple serious illnesses, who shares how Tuesday Health’s personalized, team-based care helped her regain independence, avoid hospitalizations and feel truly supported, physically and emotionally.
Before enrolling with Tuesday Health, Pam spent nearly five months hospitalized in 2021. Today, she’s walking more, biking again, and hasn’t needed a single emergency room or hospital visit since joining Tuesday Health through her CareSource health plan.
“My Tuesday Health nurse gives me more of a detailed explanation of my illnesses. I can call or text her anytime. She’s got me more motivated,” Pam shares in the video.
Tuesday Health began serving CareSource members in July 2024, delivering early palliative care designed to reduce unnecessary hospital use, improve quality of life and provide comprehensive support for individuals living with serious illness and their caregivers.
