Support for Your Most Complex Patients, Without Disruption to Your Practice

Tuesday Health partners with primary care and specialty providers to deliver high-quality palliative care that improves outcomes and complements your existing care relationships.

Flexible, High-Touch Support for Patients with Serious Illness

Your patients with advanced illness face complex medical, emotional, and social challenges, and they need more support than the traditional care model provides. Tuesday Health brings expert, value-based palliative care directly to these patients through in-person visits, video appointments, check-in calls, and 24/7 clinical support.

We work alongside you and your team to ensure patients receive consistent, goal-aligned care while keeping you informed and involved. We act as an extension of your practice, not a replacement.

When to Refer to Tuesday Health

We’re here to support your most complex, high-risk patients, typically those with:

  • Advanced chronic or serious illness (e.g., CHF, COPD, cancer, dementia)
  • Frequent hospital or ED use
  • Functional decline or poor symptom control
  • Difficult conversations around prognosis, treatment decisions, or goals of care
  • High caregiver burden or unmet social needs

Our care is appropriate alongside curative or disease-directed treatments and can begin earlier than hospice.

What You Can Expect

  • Ongoing communication and coordination with your team
  • No disruption to existing provider-patient relationships
  • Care aligned with the patient’s values, goals, and preferences
  • Improved comfort and quality of life for your patients
