Tablet Terms and Conditions

Tuesday Health Tablet Terms and Conditions 

These Terms and Conditions govern the use of a tablet provided by Tuesday Health (“the Tablet”) for the purpose of supporting your healthcare needs through our services.

1. Purpose of the Tablet

The Tablet is solely for the purpose of accessing Tuesday Health’s services and resources. This is not a gift. The Tablet has been disabled for all other uses, such that it has no value except for your care. You may not use it for any other purpose.

2. Ownership

The Tablet always remains the property of Tuesday Health. By receiving the Tablet, you agree that you will return it to Tuesday Health if no longer needed or upon request.

3. Returning the Tablet

You are expected to return the Tablet to Tuesday Health at the conclusion of its use. 

4. Restrictions on Use

  • Data Collection: The Tablet will collect data related to your use of Tuesday Health services for the purpose of improving care and service delivery. By using the Tablet, you consent to this data collection. 
  • Not a Substitute for Emergency Services: The Tablet is not a substitute for 911 or emergency services. In the event of a medical emergency, please call 911 immediately. The Tablet should only be used for accessing Tuesday Health services and is not equipped to manage emergency situations. 
  • Telehealth: As part of your care, you and Tuesday Health may communicate through the tablet with sound and/or video. Tuesday Health cannot control who is in the room or house with you during these sessions and may not know if someone else is with you. You need to tell Tuesday Health at the beginning of a call if there is information you do not want to discuss or if you want to reschedule a visit for any reason, including to protect your privacy or medical information. Please be aware that only you can be responsible for any overhearing of medical information or communication that might occur during audio and/or visual visits. 
  • Password:   The Tablet will be password protected. You are responsible for protecting your password and not allowing others in your location to use your password to access the Tablet.

 

5. Lost or Damaged Tablets

If the Tablet is lost, stolen, or damaged beyond reasonable wear, you are responsible for covering the cost of the device. In such cases, you may be required to pay the full retail price of the Tablet. If you have any problems with the tablet or believe it is not working correctly, please notify Tuesday Health at 844-698-8373. 

6. Compliance

By accepting the Tablet, you agree to use it only as stated in these Terms and Conditions.  

7. Acknowledgment

By accepting the Tablet, you acknowledge and agree to these Terms and Conditions.
